Here is a sample of the one we use. Anyone want to share theirs? It is cropped as the rest is for pest control with all our licenses. This is on the back of all of our receipts.

If I am really concerned about a client I have them initial a point of concern. I might have her/him sign "I understand" and then put her initials. ;)

Just bumping/moving this because I have had numerous emails and PM's asking for a copy. (This is minus our pest control portion) Was just going to give it to Premium Members but feel many could benefit. Feel free to use it.

I am old, but i do remember seeing that before. I just didn't "see" much. It was really small and half of it was missing. It reminded me of Christian TV. Here's half, now pay me for the rest.
The rest is pest control. Reminds me of child daycare. Sooner or later with enough info tho you let go of the hand.

I like the form, but number 5 seems like it would cause some people to take a second look. I might add something like, "Although these conditions are rare we will exert every effort to give the best cleaning possible." I guess if it ain't broke don't fix it, but I can well imagine what would happen if I showed that to a customer before cleaning their furniture. I added the term rare, because in my experience those things are for the most part extremely rare. For example, I've only seen maybe one couch bleed, which fortunately I was able to correct, and one shrinkage since 1987 and I've cleaned a fair bit of upholstery. Been doing it since year one. I'm just throwing this out there in case anyone decides to customize the form and make any changes.
